What You'll Do7
- 1Assess referred patients to determine medical condition, functional limitations, and rehabilitation potential.
- 2Design and administer treatment programs with specific goals based on patient capacity and physician direction.
- 3Set realistic, achievable goals and document progress via evaluations, weekly notes, recertifications, and discharges.
- 4Educate patients and families on therapy methods and home programs.
- 5Collaborate with all disciplines to deliver patient-centered care.
- 6Comply with HIPAA, confidentiality, and Federal Residents' Rights.
- 7Perform additional duties as assigned by the Director of Rehabilitation.
Requirements6
- 1Graduation from an accredited Speech Therapist program.
- 2Valid professional license and registration for the state of TN.
- 3Experience with long-term care or geriatric care preferred.
- 4Skilled in documentation: evaluations, progress notes, recertification, and discharge summaries.
- 5Exceptional patient service and clinical excellence.
