What You'll Do8

  • 1Manage a daily caseload of speech therapy patients, ensuring timely care and documentation per the established plan and physician orders.
  • 2Collect patient data from home assessments, telephonic contacts, and EMR information to evaluate health status.
  • 3Evaluate patients to determine rehabilitation needs and potential for achievement, then develop a plan of care with the physician.
  • 4Implement certified speech therapy services and direct care coordination with a multidisciplinary team for effective outcomes.
  • 5Communicate schedule changes, patient updates, and physician orders with the team to ensure continuity.
  • 6Document short- and long-term goals in visit notes, progress notes, and discharge summaries.
  • 7Collaborate with physician oversight and branch supervision for timely feedback on patient condition or plan changes.
  • 8Perform treatments per Medicare Guidelines, OASIS, and TJC standards, adhering to the Licensure Act for Speech Language Pathologists.

Requirements8

  • 1Master's Degree in Speech Language Pathology and current state licensure required.
  • 2BLS certification required per policy guidelines.
  • 31+ years of home health experience preferred, but new graduates with clinical rotations considered.
  • 4Ability to lift and support 50 pounds during patient handling and equipment transport.
  • 5Willingness to travel extensively across seasons to patient residences in the Monroe area.
  • 6Proficiency in EMR charting and documentation.
  • 7Critical thinking skills to respond to changing patient conditions and flexible scheduling to meet patient needs.
  • 8Demonstrated communication skills for coordinating with physicians, patients, and interdisciplinary teams.
