What You'll Do8
- 1Perform thorough evaluations to assess, diagnose, and plan therapeutic interventions for patients with speech, language, communication, or swallowing difficulties in home settings.
- 2Develop personalized treatment plans based on evaluations, physician orders, and recommended goals, adjusting them as patients progress.
- 3Revise speech therapy care plans and participate with BAYADA clinicians in multidisciplinary care planning to ensure holistic patient care.
- 4Educate patients, family members, and caregivers on rehabilitative care and activities that promote health, safety, and independent living.
- 5Document all evaluations, interventions, and observations accurately using a state-of-the-art touch pad tablet for EMR.
- 6Coordinate with the care team and physicians to ensure seamless communication and continuity of care.
- 7Manage a caseload across designated geographic territories, optimizing visit schedules for patient outcomes.
- 8Stay current with SLP best practices and BAYADA protocols to deliver high-quality care.
Requirements7
- 1Current SLP license in North Carolina.
- 2Possess a certificate of clinical competence in speech from the American Speech and Hearing Association (or be eligible to take the exam).
- 3Have completed the educational and work requirements for certification and be in the process of accumulating supervised work experience.
- 4Prior home care experience as a Speech Language Pathologist is a plus.
- 5Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- 6Ability to travel to patients' homes within Franklin and Granville counties.
- 7Reliable transportation and valid driver's license.
