Speech Therapist SLP Home Health Houston
Overview
You will own the full therapy cycle for home health patients across North Houston and The Woodlands: evaluate, treat, and track progress in communication, cognition, and swallowing disorders. You work one-on-one with adults recovering from stroke, brain injury, or neurological conditions, delivering SLP interventions in the patient's home. What You'll Do10
- 1Evaluate speech, language, communication, cognitive, voice, and swallowing disorders for home health patients.
- 2Develop individualized treatment plans based on physician orders and patient needs.
- 3Provide evidence-based therapeutic interventions to improve communication and swallowing function.
- 4Assess and treat patients with speech, language, cognitive, and dysphagia challenges from illness, injury, or neurological conditions.
- 5Educate patients, family members, and caregivers on treatment strategies and home exercise programs.
- 6Recommend communication aids, adaptive devices, and compensatory strategies as appropriate.
- 7Monitor patient progress and modify treatment plans to achieve therapeutic goals.
- 8Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care.
- 9Communicate changes in patient condition and treatment response to the physician and Director of Nursing.
- 10Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner.
Requirements8
- 1Master's Degree in Speech-Language Pathology from an accredited program.
- 2Current Speech-Language Pathologist license in the state where services are provided.
- 3Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred or required based on state regulations.
- 42+ years of experience as a Speech-Language Pathologist.
- 5Home health or community-based experience preferred.
- 6Experience evaluating and treating patients with communication, cognitive, and swallowing disorders.
- 7Strong clinical assessment, communication, and documentation skills.
- 8Commitment to maintaining HIPAA compliance and patient confidentiality.
Salary Insight
$70 - $75k per year
