What You'll Do9

  • 1Conduct comprehensive evaluations of speech language cognitive and swallowing function
  • 2Develop and implement individualized treatment plans to improve communication cognition and safety
  • 3Utilize evidence-based techniques and specialized tools for therapeutic interventions
  • 4Assess home environments and recommend adaptive equipment or communication aids
  • 5Educate patients and caregivers on exercises compensatory strategies and safety techniques
  • 6Monitor patient progress reassess treatment plans and document findings in the EMR
  • 7Identify clinical changes and communicate proactively with physicians and care teams
  • 8Collaborate with nurses and interdisciplinary teams for coordinated care
  • 9Participate in case conferences discharge planning and ongoing care coordination

Requirements11

  • 1Master`s or Doctoral degree in Speech-Language Pathology
  • 2Current SLP license in the state of practice
  • 3Minimum of 1 year clinical experience in home health settings
  • 4Certificate of Clinical Competence (CCC-SLP) preferred
  • 5Strong clinical judgment and ability to work independently
  • 6Excellent communication and patient education skills
  • 7Comfortable with field-based work including travel between patient homes
  • 8Valid CPR certification
  • 9Reliable driver’s license and auto insurance
  • 10Experience in home health or community-based care
  • 11Confidence managing dysphagia and cognitive-communication disorders
