What You'll Do11
- 1Assess patient physical psychological social spiritual educational developmental cultural discharge needs and complete evaluations within required timeframe
- 2Formulate individualized care plans with family incorporating assessment data and multidisciplinary input
- 3Implement evidence-based interventions aligned with organizational policies and professional standards
- 4Coordinate with physicians pharmacists therapists and other specialists to ensure seamless care delivery
- 5Document assessments plans and evaluations accurately and promptly in accordance with policy
- 6Supervise staff and manage workflow while maintaining high clinical performance metrics
- 7Participate in unit leadership initiatives and share expertise through precepting and mentoring
- 8Contribute to research activities and professional development opportunities
- 9Advocate for patient-centered care that promotes timely discharge and optimal outcomes
- 10Collaborate with external partners including community providers and clergy to address patient needs
- 11Analyze patient data and synthesize findings to inform clinical decision-making
Requirements7
- 15+ years experience in acute care nursing with telemetry ICU or critical care backgrounds
- 2Current BLS certification and valid RN license in Arizona
- 3Proficiency with Spark and Airflow for data pipeline development
- 4Minimum one year of experience in acute care hospital settings
- 5Bachelor's degree preferred along with relevant certifications
- 6Strong communication skills for interdisciplinary collaboration
- 7Commitment to continuous learning and professional growth
