Overview
PACE Referral Tracking Coordinator I processes medical referrals from NEMS providers ensuring eligibility verification prior authorization and documentation in Epic. This full-time onsite role tracks referrals and ensures compliance with turnaround times while collaborating with providers and hospitals. It differs by focusing on end-to-end referral management and proactive follow-up.
What You'll Do7
- 1Process referrals by verifying insurance eligibility and submitting complete packets including prior authorizations
- 2Track referral status and obtain consultation reports to close loops
- 3Follow up on outstanding referrals and contact care teams about patient needs
- 4Provide PACE care teams with detailed referral information and specialty provider details
- 5Enter all referral activities and communications into the Electronic Medical Record
- 6Maintain accurate documentation and follow directions with minimal supervision
- 7Perform other duties as required by supervisor
Requirements5
- 1High school degree or equivalent AA preferred with 2 years customer service or healthcare experience
- 2Strong communication skills both written and verbal for system documentation
- 3Knowledge of insurance types such as Medi-Cal Medicare and private insurance
- 4Fluency in English mandatory with Cantonese or Mandarin preferred
- 5Attention to detail and excellent data entry abilities
