PACE Scheduler
Overview
We seek an organized customer-focused PACE Scheduler to deliver administrative and scheduling support for the PACE Center under the Operations Director. You will coordinate participant appointments manage transportation ensure provider documentation accuracy enter scheduling information into electronic medical records communicate appointment details with participants families and staff collaborate with interdisciplinary teams respond to scheduling changes provide coverage for assistants maintain records and follow procedures.
What You'll Do9
- 1Schedule participant appointments with community providers and specialists
- 2Coordinate transportation for scheduled appointments
- 3Ensure providers receive required documentation before appointments
- 4Accurately enter scheduling information into electronic medical records
- 5Communicate appointment information with participants families and providers and PACE staff
- 6Collaborate with the Interdisciplinary Team clinic staff and leadership
- 7Respond promptly to appointment requests and scheduling changes
- 8Provide coverage for Care Team Assistants as directed
- 9Maintain accurate records and follow established scheduling procedures
Requirements6
- 1High school diploma or GED or equivalent combination of education and related experience
- 2Strong verbal and written communication skills
- 3Excellent organization documentation and time-management abilities
- 4Proficiency in Microsoft Word Excel and Outlook
- 5Ability to multitask prioritize and work independently with multiple teams
- 6Healthcare scheduling or administrative experience preferred
