Transportation Coordinator, Senior Care & PACE Programs
Overview
You will coordinate all transportation for Gary and Mary West PACE participants, ensuring timely, safe access to the PACE center and critical medical appointments. Working within a supportive, mission-driven team, you'll serve as the key liaison among participants, caregivers, staff, and contracted vendors. What You'll Do10
- 1Participate in IDT meetings, communicating transportation updates and participant needs to support coordinated care.
- 2Coordinate and maintain participant transportation schedules with contracted vendors for the PACE center, specialty appointments, and dialysis.
- 3Serve as the primary point of contact for scheduling changes, cancellations, delays, and participant inquiries.
- 4Handle same-day transportation changes and urgent requests, updating all parties promptly.
- 5Use EMR and scheduling software to document activities, communications, and updates per PACE regulations.
- 6Perform reminder calls for transportation and appointments, logging interactions in the EMR.
- 7Discontinue transportation services for disenrolled or deceased participants in a timely manner.
- 8Monitor and maintain accurate schedules, manifests, and operational data to support daily reporting needs.
- 9Distribute daily schedules to vendors and staff, ensuring timely dissemination.
- 10Interact professionally with elderly participants, including those with cognitive or physical impairments, maintaining confidentiality and HIPAA compliance.
Requirements10
- 1High school diploma or GED required.
- 21+ year of scheduling, transportation coordination, medical office, or related healthcare experience preferred.
- 31+ year documented experience working with a frail or elderly population required.
- 4Experience with EMR and transportation or scheduling software preferred.
- 5Proficient in Microsoft Office (Outlook, Word, Excel).
- 6Bilingual English/Spanish strongly preferred.
- 7Excellent organizational skills to manage multiple priorities in a fast-paced setting.
- 8Strong verbal and written communication skills to interact with diverse stakeholders.
- 9Ability to exercise sound judgment, prioritize urgent needs, and resolve conflicts.
- 10Ability to work independently and collaboratively within an interdisciplinary team.
