Social Worker, PACE Elder Care and Interdisciplinary Team
Overview
As a Social Worker at the Center for Elders' Independence, you will own psychosocial assessments, care planning, and crisis intervention for a caseload of frail elderly participants in a PACE program. You will join an interdisciplinary team of clinicians, psychiatrists, and community partners, providing integrated care that keeps elders living independently. What You'll Do9
- 1Serve as a core member of the interdisciplinary team, developing and implementing comprehensive care plans that address psychosocial needs and monitor progress.
- 2Conduct initial and ongoing psychosocial assessments, re-assessments, and care plan updates for assigned participants, documenting accurately and timely.
- 3Lead 30-day conferences, annual reviews, and family/caregiver meetings to align on goals and interventions.
- 4Make referrals to community resources for financial management, benefit eligibility, and other participant needs, coordinating with external agencies.
- 5Investigate and report complaints of elder abuse, ensuring participant safety and compliance with regulations.
- 6Manage an assigned caseload, preparing narrative reports, case files, and correspondence regarding client status.
- 7Participate in behavioral management meetings and psychosocial rounds with the consulting psychiatrist, presenting cases as needed.
- 8Build and maintain relationships with community agencies, including psychiatric facilities, hospitals, and social services, to support discharge planning and continuity of care.
- 9Conduct home visits and travel to meet participants and service providers, maintaining confidentiality per HIPAA guidelines.
Requirements8
- 1Master's degree in social work from an accredited school.
- 2One year of experience working with frail or elderly populations preferred.
- 3Demonstrated ability to work in a multicultural, multidisciplinary team environment.
- 4Ability to work with minimal supervision and excellent oral and written communication skills.
- 5Experience with severe mental illness is desirable.
- 6Bilingual ability in Spanish, Korean, Cantonese, or Mandarin preferred.
- 7Valid California driver's license, acceptable driving record, and insurance with access to reliable transportation.
- 8Current CPR and First Aid certification.
