Bilingual Social Worker, Aging Case Management
Overview
You own a caseload of 35-40 aging adults, guiding them through hospital-to-home transitions and long-term community support. Your daily work uses Medicare, Medi-Cal, and local resources to prevent premature facility placement. You join a team of case managers, nurses, and social workers at Catholic Charities, reporting to the Program Director. You deliver culturally competent care with respect for diverse backgrounds and languages. What You'll Do10
- 1Manage a caseload of 35-40 clients, completing comprehensive screenings and assessments within 24-48 hours of intake.
- 2Build client care plans centered on their goals, adjusting them as needs evolve through quarterly visits and monthly check-ins.
- 3Connect clients to Medicare, Medi-Cal, and other entitlement programs, arranging and monitoring community services.
- 4Provide crisis intervention, advocacy, and therapeutic counseling to stabilize clients in their homes.
- 5Document all client interactions and care plan updates in CARES Data Systems within 24-48 hours to ensure continuity.
- 6Educate clients and families on available resources, training them to advocate for themselves successfully.
- 7Coordinate with physicians, health workers, and community providers to align care and services.
- 8Evaluate each client's safety at home, planning transitions to facilities only when necessary.
- 9Lead discharge planning, documenting the process in compliance with Case Management Accredited guidelines.
- 10Participate in team meetings, peer reviews, and trainings to improve quality of care and service delivery.
Requirements10
- 1Master's degree in Social Work or a related field.
- 23+ years of relevant experience, including 2+ years in case management with aging populations.
- 3Bilingual in Mandarin, Cantonese, Tagalog, or Spanish.
- 4Certified in Red Cross First Aid and CPR (or obtainable within 30 days of hire).
- 5Knowledge of community resources and entitlement programs for the elderly.
- 6Computer literacy in Microsoft Office and data entry systems.
- 7Exceptional communication and problem-solving skills with frail elderly clients.
- 8Ability to work under pressure and maintain confidentiality.
- 9Sensitivity to diverse cultures and aging issues.
- 10Physical ability to lift up to 20 pounds and perform frequent bending, standing, and walking.
