Social Services Assistant, Skilled Nursing & Resident Care
Overview
You own social services support at a 120+ bed skilled nursing facility, ensuring resident care plans, Medi-Cal applications, and discharge coordination meet state and federal regulations. You coordinate with Case Managers, Nursing staff, and families to resolve grievances and manage room changes. This role drives compliance with HIPAA privacy rules and company policy. You will lead the Social Services department in facilities under 99 beds, or support the Director in larger settings.
What You'll Do7
- 1Complete psychosocial assessments and care plans for residents within the first 90 days, integrating Medi-Cal eligibility and transport needs.
- 2Coordinate care plan meetings with families on admission and quarterly, ensuring all discharge plans are updated and executed.
- 3Monitor psychotropic medication consent and documentation, assisting Nursing staff with resident reviews.
- 4Resolve resident and family grievances, documenting actions per state and federal regulations.
- 5Coordinate room changes with Case Managers and facility staff, ensuring smooth transitions.
- 6Complete required forms and documentation for Medi-Cal and other programs, meeting all compliance deadlines.
- 7Oversee facility efforts to protect resident health information, training staff on HIPAA privacy practices.
Requirements7
- 1Social Services designee approved program or certificate from a Community College for facilities of 99 beds or less.
- 21+ years experience in long-term care or skilled nursing settings.
- 3Knowledge of Medi-Cal application processes and discharge planning procedures.
- 4Familiarity with HIPAA regulations and resident rights.
- 5Ability to lead a department or support a Director in facilities of 120+ beds.
- 6Proficiency in Microsoft Office for documentation and reporting.
- 7Effective communication with residents, families, and interdisciplinary teams.
