Social Service Director, Long-Term Care & Discharge Planning
Overview
You will lead psychosocial care and discharge planning for short-term rehab and long-term care guests at Symphony Applewood. You own resident assessments, care plans, and transitions, impacting 50+ residents and their families. You collaborate daily with nursing, physicians, and hospice teams, with a focus on compassionate advocacy. This contract role offers daily pay and tuition reimbursement, with growth through Symphony Care Network.
What You'll Do7
- 1Conduct psychosocial and cognitive assessments for new admissions within 48 hours.
- 2Develop and maintain resident care plans, updating them with the interdisciplinary team each quarter.
- 3Drive discharge planning, coordinating with families and community providers for smooth transitions.
- 4Provide emotional support and advocacy for residents and families during care conferences.
- 5Collaborate with nursing and physicians to align care goals and address psychosocial barriers.
- 6Document assessments and interventions accurately in PointClickCare.
- 7Lead end-of-life, hospice, and palliative care discussions with empathy and clarity.
Requirements7
- 1Bachelor’s Degree in social work, psychology, or related field required.
- 22+ years of experience in long-term care and short-term rehabilitation preferred.
- 3Proven conflict resolution and problem-solving skills in resident care settings.
- 4Excellent communication and interpersonal abilities with families and staff.
- 5Working knowledge of federal and state regulations for long-term care, including CMS standards.
- 6Strong organizational and time management skills to manage caseloads and deadlines.
- 7PointClickCare experience preferred, with proficiency in documentation and care coordination.
