What You'll Do10
- 1Greet new residents and orient them to facility staff

- 3Write quarterly and annual social assessments

Requirements6
- 1Licensed Clinical Social Worker certification
- 25+ years experience in long-term care social services
- 3Understanding of JCAHO OBRA IDPH HFS regulations
- 4Ability to perform discharge planning and policy adherence
- 5Knowledge of psychosocial dynamics of geriatric population
- 6Strong communication and empathy for residents families and staff
