What You'll Do10

  • 1Assist residents and families through facility admission, addressing transition challenges and providing emotional support.
  • 2Complete comprehensive psychosocial assessments for each resident, documenting social, emotional, and psychological needs and strengths.
  • 3Identify medically related social needs, then coordinate appropriate services such as counseling, support groups, or community referrals.
  • 4Compile resident social information into personal files upon admission, ensuring accurate and confidential records.
  • 5Maintain a current list of facility and community resources that support social services and clinical care.
  • 6Plan, organize, and implement social service programs that enhance resident quality of life and meet regulatory standards.
  • 7Evaluate program effectiveness, collaborating with the Social Services Director, Administration, and facility staff to adopt best practices.
  • 8Assess each resident for discharge, coordinating with families and community providers to ensure seamless transitions.
  • 9Advocate for residents, ensuring their best interests guide all social service and clinical care decisions.
  • 10Conduct resident interviews to complete MDS assessments in cognitive, mood, behavior, and goal setting, along with Care Area Assessments.

Requirements7

  • 1Master's or Bachelor's degree in human services, social work, psychology, or a related field.
  • 2Long-term care experience preferred, with knowledge of skilled nursing facility regulations.
  • 3Ability to perform psychosocial assessments and develop care plans.
  • 4Strong interpersonal and communication skills for resident, family, and team interactions.
  • 5Familiarity with MDS documentation and discharge planning processes.
  • 6Commitment to resident advocacy and ethical practice.
  • 7Ability to work onsite at the facility in Philadelphia.
