As a Part-Time Social Worker at Epic Rehabilitation & Nursing at White Plains, you will own resident advocacy and psychosocial care for a 120-bed skilled nursing facility in White Plains, NY. You will join a multidisciplinary team of nurses, therapists, and administrators, reporting to the Director of Social Services. What You'll Do10

  • 1Conduct admission interviews within 72 hours to help new residents adjust to the facility and overcome relocation trauma.
  • 2Compile social and psychosocial histories from referral sources, families, and residents within 14 days of admission.
  • 3Develop and document initial psychosocial assessments and care plans in the medical record, meeting all regulatory deadlines.
  • 4Assess discharge potential for every resident and specify discharge plans in the care plan to ensure smooth transitions.
  • 5Serve as a liaison between residents, families, and the interdisciplinary team to address grievances and resolve concerns.
  • 6Educate staff on the psychosocial, economic, and cultural factors affecting resident behavior and functioning.
  • 7Review and update psychosocial assessments and care plans every 90 days and as needed based on resident condition changes.
  • 8Advocate for resident rights, encouraging them to voice complaints and participate in care decisions.
  • 9Collaborate with the psychologist consultant and other departments to coordinate mental health support.
  • 10Maintain accurate and timely documentation in compliance with state and federal regulations.

Requirements10

  • 1Bachelor's degree in Social Work or a related field.
  • 22+ years of experience in social services, preferably in a nursing facility, hospital, or related healthcare setting.
  • 3Demonstrated ability to work with elderly, disabled, and emotionally challenged residents with empathy and patience.
  • 4Proficiency in Microsoft Office (Word, Excel, Outlook) for documentation and communication.
  • 5Excellent verbal and written communication skills for interacting with residents, families, and interdisciplinary teams.
  • 6Strong organizational skills to manage multiple cases and meet documentation deadlines.
  • 7Ability to work under pressure in a fast-paced clinical environment, maintaining flexibility and composure.
  • 8Willingness to collaborate harmoniously with all staff and departments.
  • 9Completion of a Covid-19 vaccination is encouraged but not mandated per CDC guidelines.
