Overview

You'll own resident psychosocial care at a skilled nursing community in Arlington Heights, IL, managing assessments, care plans, and family support for a census of older adults. You'll partner with nursing, therapy, and chaplaincy to coordinate discharges and connect residents with community resources. Two openings exist on a team that values resident advocacy and interdisciplinary collaboration. This full-time role offers PointClickCare training, daily pay, and 24/7 chaplain support, making a real difference in residents' transitions.

What You'll Do7

  • 1Complete social service assessments for new and current residents, identifying psychosocial needs and preferences within the first week.
  • 2Coordinate discharge plans with families and outside providers, ensuring seamless transitions to home or other facilities.
  • 3Advocate for resident rights and support families through care transitions, including difficult conversations.
  • 4Facilitate family involvement in care planning meetings, ensuring voices are heard in all decisions.
  • 5Connect residents with community resources and encourage participation in facility programming to boost engagement.
  • 6Document all social service plans, progress notes, and assessments in PointClickCare accurately and timely.
  • 7Collaborate with nursing, therapy, and care team on resident care plans, integrating social work goals.

Requirements5

  • 12+ years of experience in a social services role within a senior living setting.
  • 22+ years of discharge planning experience in senior living.
  • 31+ year of hands-on experience using PointClickCare (PCC).
  • 4Bachelor's degree in social work, human services, or related field (preferred).
  • 5Experience working with Medicare/Medicaid resident populations (preferred).
