Case Manager, Bilingual Social Services in Bronx
Overview
You will own case management for tenants in a Bronx program serving individuals and families, using English and Spanish to coordinate care. You will report to the social services director and collaborate with a team of Social Workers and Case Aides. What You'll Do10
- 1Build and maintain tenant files, reviewing all eligibility documentation and creating organized records.
- 2Conduct initial intake and assessments of tenant needs, then perform periodic reassessments to track progress.
- 3Design personalized independent living plans with tenants, setting short-term and long-term goals.
- 4Locate community resources and make referrals to internal and external services, scheduling appointments as needed.
- 5Coach tenants to overcome barriers, advocating for themselves and moving toward self-sufficiency.
- 6Monitor tenant progress through regular phone or in-person visits, documenting each contact in progress notes.
- 7Participate in case conferences, sharing updates and coordinating care with other professionals.
- 8Drive program reporting by compiling data for weekly, monthly, quarterly, and annual summaries.
- 9Evaluate tenant stability for case closure, recommending when goals are met, participation wanes, or eligibility ends.
- 10Input tenant data and progress updates into the automated database, ensuring accuracy and confidentiality.
Requirements7
- 1Bachelor's degree in social work or related field, with experience supporting SPMI populations preferred.
- 2Bilingual fluency in English and Spanish required for client communication and documentation.
- 31+ years of case management experience, including intake, assessment, and referral coordination.
- 4Computer literacy in Microsoft Office Suite (Word, Excel, Outlook) for case notes and reports.
- 5Strong written and verbal communication skills to collaborate with tenants, families, and agencies.
- 6Fingerprint clearance and Criminal History Check from the Justice Center required, maintained throughout employment.
- 7Permanent US work authorization without visa sponsorship now or in the future.
