Care Coordinator Spanish Bilingual Job in NYC | Good Shepherd
Overview
You will coordinate care for youth in foster care, guiding families through health systems and ensuring access to services. Reporting to the Care Manager Supervisor, you will work in a team serving 30,000+ children annually. This hybrid role in the Bronx requires bilingual Spanish and uses the CANS assessment and MAPP portal. You will build relationships with providers and drive outcomes for vulnerable youth, growing within a top NYC workplace.
What You'll Do10
- 1Secure required Care Management enrollment consents from youth or guardians within the first 30 days.
- 2Conduct initial and ongoing CANS assessments to determine care levels and update plans quarterly.
- 3Develop and manage Individualized Plans of Care (IPC), coordinating services across medical, behavioral, and social domains.
- 4Complete face-to-face visits per acuity level, ensuring compliance with high, medium, or low requirements.
- 5Document all care activities in MAPP and EHRs within required timeframes.
- 6Advocate for clients with external providers and connect them to benefits like Medicaid, SNAP, and housing.
- 7Educate families on chronic condition management, immunizations, and preventive screenings.
- 8Facilitate care planning meetings and collaborate with interdisciplinary teams for holistic care.
- 9Manage referrals and follow-up for community resources, and participate in hospital discharge planning.
- 10Ensure children receive dental, vision, and hearing screenings per EPSDT guidelines.
Requirements7
- 12+ years of experience in social services, including case planning, care coordination, or case management.
- 2Bachelor's degree in arts or science plus two years of relevant experience.
- 3Bilingual Spanish and English proficiency is required.
- 4Prior experience as a Case Planner, Care Coordinator, Case Manager, or Case Worker is required.
- 5Experience with Child Welfare, ACS, and Foster Care is helpful.
- 6Demonstrated knowledge of serving children in child welfare, mental health, or healthcare systems.
- 7Experience providing service coordination, information, and referrals for community-based services.
