Overview

You will coordinate comprehensive, family-driven care for children and adolescents with complex medical, behavioral health, developmental, and social needs at AthenaPsych. You will develop individualized Plans of Care, coordinate services across systems, and advocate for access to community resources. Collaborating closely with families, providers, schools, and community agencies, you ensure integrated care across medical, behavioral health, and social systems. This role emphasizes strengths-based, trauma-informed, and culturally responsive planning, supporting youth in achieving optimal health, stability, and functioning in the community.

What You'll Do10

  • 1Conduct comprehensive assessments to identify strengths, needs, and service gaps for youth on your caseload within the first 30 days.
  • 2Develop, implement, and monitor individualized, family-driven Plans of Care in partnership with families and interdisciplinary teams.
  • 3Coordinate medical, behavioral health, educational, and social services to ensure seamless care integration.
  • 4Facilitate care team meetings and interdisciplinary collaboration to align on treatment goals and interventions.
  • 5Support transitions between levels of care or service systems, ensuring continuity and safety.
  • 6Provide outreach and ongoing engagement with youth and caregivers to maintain consistent participation.
  • 7Educate families on available services, benefits, and treatment options to empower informed decisions.
  • 8Advocate for access to medical, behavioral health, social, educational, and supportive services for each youth.
  • 9Maintain accurate, timely documentation of services, assessments, and care plans in compliance with AthenaPsych standards.
  • 10Conduct home visits, school visits, and community-based meetings to support youth in their natural environments.

Requirements8

  • 1Master's degree in Social Work, Psychology, Human Services, Counseling, or related field required.
  • 22+ years of experience working with children, adolescents, or families in behavioral health, social services, or care management.
  • 3Experience in care coordination or case management programs, with a strong understanding of child welfare and developmental disabilities systems.
  • 4Knowledge of behavioral health systems and community resources for youth.
  • 5Strong communication, organization, and problem-solving skills for multidisciplinary collaboration.
  • 6Ability to work independently and collaboratively with multidisciplinary teams.
  • 7Bilingual in Spanish preferred.
  • 8Valid driver's license and access to reliable transportation for home and school visits.
