Care Coordinator Community
Overview
Lead bilingual Spanish-speaking Care Coordinators to support youth in the Health Home program. Guide participants and guardians through healthcare systems while addressing access barriers and fostering provider relationships. Ensure high-quality care coordination. Reports to CARE MANAGER SUPERVISOR HEALTH SERVICES located at 685 E. 187th Street Bronx NY.
What You'll Do15
- 1Secure Care Management enrollment consents from individuals or guardians
- 2Conduct comprehensive needs assessments using Child and Adolescent Needs and Strengths tool
- 3Develop and manage Individualized Plans of Care aligned with acuity levels
- 4Coordinate services across prevention wellness medical behavioral health transition and social domains
- 5Perform face-to-face visits for child-serving coordinators based on acuity
- 6Complete accurate documentation via Care Management Portal and EHRs
- 7Advocate for clients within agency and external providers
- 8Promote wellness by connecting enrollees to resources
- 9Educate children and caregivers on chronic condition management and preventive health
- 10Assist with public benefit applications including Medicaid and housing supports
- 11Facilitate care planning meetings and interdisciplinary collaboration
- 12Identify community resources and manage referrals and follow-ups
- 13Participate in hospital discharge planning and post-release services
- 14Attend staff development and training sessions
- 15Ensure dental vision and hearing evaluations per Medicaid EPSDT guidelines
Requirements6
- 1Bachelor of Arts or Science degree with two years experience required
- 2Bilingual Spanish English speaking preferred
- 3Experience as Case Planner Care Coordinator Case Manager or Case Worker in social services
- 4Proven experience with Child Welfare ACS and Foster Care systems
- 5Additional experience serving high acuity enrollees or children with medical fragility
- 6Strong communication skills sensitive to language literacy and cultural preferences
