Care Coordinator Community Engagement Role
Overview
Coordinate client care and foster community partnerships to ensure continuity of services. This position leads engagement efforts and builds relationships with schools healthcare providers and community organizations. Responsibilities include managing caseloads developing care plans connecting clients to resources monitoring progress and facilitating communication between stakeholders. The role emphasizes relationship stewardship without sales quotas.
What You'll Do11
- 1Coordinate and manage services for an assigned caseload of clients in outpatient behavioral health
- 2Develop, implement, and monitor individualized care plans in collaboration with clinical staff
- 3Connect clients and families to appropriate community resources such as schools social services and healthcare providers
- 4Monitor client progress engagement and barriers to care
- 5Facilitate communication between clients families and multidisciplinary providers
- 6Support transitions of care including hospital discharge to outpatient services
- 7Maintain timely accurate and compliant documentation in accordance with organizational and payer requirements
- 8Serve as primary point of contact for assigned referral partners
- 9Conduct weekly check-ins with key partners to support coordination of care
- 10Provide occasional program education to referral sources through presentations emails and service updates
- 11Track referral activity and maintain basic awareness of referral patterns and partner needs
Requirements5
- 1Bachelor’s degree in social work psychology counseling or related field required with Master’s preferred
- 2Minimum of 1–2 years of experience in behavioral health case management or care coordination
- 3Knowledge of community resources and service systems preferably in Texas
- 4Experience working with children adolescents and/or families especially in school-based settings
- 5Strong organizational communication and interpersonal skills
