Overview

The Care Coordinator will lead efforts to improve health care access for youth in the Health Home program. This role reports to the CARE MANAGER SUPERVISOR, HEALTH SERVICES and works from 503 Fifth Avenue, Brooklyn, NY. Candidates should be bilingual Spanish-English speakers committed to delivering compassionate support.

What You'll Do14

  • 1Secure all required Care Management enrollment consents from participants or legal guardians
  • 2Conduct initial and ongoing comprehensive needs assessments using Child and Adolescent Needs and Strengths tool
  • 3Develop and implement Individualized Plans of Care aligned with acuity levels
  • 4Coordinate services across prevention wellness medical behavioral health transition and social domains
  • 5Complete accurate Care Management documentation using Medicaid Analytics Performance Portal and EHRs
  • 6Advocate for clients within the agency and external providers
  • 7Promote wellness by connecting enrollees to community resources
  • 8Provide education on chronic condition management immunizations and screenings
  • 9Assist with obtaining public benefits including Medicaid SNAAP and housing support
  • 10Facilitate care planning meetings and interdisciplinary team collaboration
  • 11Identify manage referrals and coordinate access to community-based services
  • 12Participate in hospital discharge planning and ensure post-release services are arranged
  • 13Attend staff development and training sessions
  • 14Ensure dental vision and hearing evaluations per Medicaid EPSDT guidelines

Requirements9

  • 15+ years experience in case planning or care coordination within social services settings
  • 2Bachelor of Arts or Science degree with two years experience required
  • 3Bilingual Spanish English proficiency preferred
  • 4Experience with Child Welfare ACS and Foster Care systems beneficial
  • 5Proven ability to work with medically fragile populations
  • 6Demonstrated experience in care transitions and community resource linkage
  • 7Strong communication skills sensitive to cultural and linguistic diversity
  • 8Knowledge of Medicaid programs and eligibility requirements
  • 9Ability to manage multiple caseloads while maintaining accuracy and compliance
