Overview
Acacia Network seeks a dedicated Care Manager for Children’s Health Home to lead advocacy and care coordination for at‑risk youth. What You'll Do11
- 1Provide direct coaching, education and advocacy to link, engage and retain clients in their Plan of Care
- 2Escort clients to appointments and gather critical information to promote wellness and reduce adverse events
- 3Elicit provider support and coordinate communication through multiple channels
- 4Conduct outreach to identify and locate potential clients within target neighborhoods
- 5Deliver intensive care management for chronic illnesses and advocate for full service access
- 6Complete required assessments such as Comprehensive Assessments and CANS‑NY evaluations
- 7Develop and review SMART goals within individualized care plans
- 8Maintain chart compliance and submit daily activity logs according to policy
- 9Coordinate with Health Navigator and outreach teams to refer members to specialized services
- 10Participate in quality improvement initiatives and prepare program reports
- 11Attend regular supervision and staff meetings to discuss performance and operational issues
Requirements8
- 1Bachelor’s Degree in social work psychology nursing rehabilitation education occupational therapy physical therapy recreation counseling community mental health child and family studies sociology speech and hearing or other human services fields preferred
- 2Two (2) years’ experience in community outreach or volunteer coordination preferred
- 3Valid Mandated Reporter certification obtained before hire date
- 4Excellent public speaking writing and interpersonal skills
- 5Highly organized self‑starter with strong time management abilities
- 6Bilingual Spanish proficiency a plus
- 7Ability to analyze data and produce narrative or quantitative reports
- 8Experience with ePaces eligibility verification and field documentation
