Case Manager SEBCO
Overview
The Bronx Men’s Homeless Shelter seeks a compassionate Case Manager to help residents transition to permanent housing. This role involves managing resident cases and supporting them toward self‑sufficiency. The ideal candidate is dedicated and experienced within the DHS/OTDA system.
What You'll Do11
- 1Conduct initial resident intakes and enter accurate information into CARES within 48 hours
- 2Establish professional relationships with residents while maintaining confidentiality
- 3Create and update Independent Living Plans weekly and document daily service notes
- 4Complete Psycho‑Social Assessments in CARES within the first month and annually thereafter
- 5Schedule appointments with referral agencies and follow up on participation
- 6Monitor and document client engagement weekly
- 7Act as liaison advocating for residents with external organizations
- 8Assist residents in securing Public Assistance benefits
- 9Escort residents to appointments and obtain required documentation such as NY State IDs and birth certificates
- 10Provide employment support including resume preparation and job referrals
- 11Attend staff meetings, trainings, and professional development activities
Requirements11
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons leading case management teams
- 35+ years experience with DHS/OTDA system
- 4Bachelor’s degree in Social Work or related field
- 5Strong computer proficiency with Microsoft Office and databases
- 6Excellent verbal and written communication skills
- 7Analytical and organizational abilities
- 8Time‑management and documentation accuracy
- 9Ability to work effectively with diverse populations
- 10Bilingual English/Spanish is a plus
- 11Knowledge of homeless services and public assistance programs
