Case Manager, Social Services & Trauma-Informed Care
Overview
Case Manager owns a caseload of 25 families at a supportive hotel program in New York City. You will deliver trauma-informed services using evidence-based models like motivational interviewing and person-centered planning. Join a social justice focused team at Goddard Riverside, part of the Integrated Support Hotel Program. You will advocate for families, coordinate with NYC DOE, and help them secure permanent housing.
What You'll Do11
- 1Manage a caseload of 25 families, conducting weekly contacts to monitor goal compliance.
- 2Perform culturally sensitive intakes, assessments, and service planning using motivational interviewing and trauma-informed care.
- 3Advocate for families to access community resources including medical, dental, and mental healthcare services.
- 4Coordinate interpretation and translation services for diverse clients.
- 5Collaborate with shelter staff to enforce rules and regulations, ensuring a safe environment.
- 6Troubleshoot with NYC DOE on student registration, school placement, and attendance issues.
- 7Refer families to childcare and community-based services as needed.
- 8Maintain accurate client case records in CARES and other databases.
- 9Assist with residential groups and activities to support community building.
- 10Participate in staff meetings, case conferences, and program evaluation.
- 11Attend trainings and supervision to enhance service delivery.
Requirements10
- 1Bachelor’s degree in social work or human services field, strongly preferred.
- 21+ years of case management experience in a shelter or congregate housing setting.
- 3Strong understanding of trauma and delivering trauma-informed, culturally sensitive services.
- 4Excellent verbal and written communication skills for diverse populations.
- 5Strong organizational skills with ability to handle multiple priorities and resolve conflicts.
- 6Proficiency in Microsoft Office (Word, Excel, Outlook) required.
- 7Ability to learn database systems such as CARES.
- 8Bilingual in Spanish or other languages preferred.
- 9Physical ability to walk, stand, and lift up to 30 lbs.
- 10Willingness to work in all weather conditions for field visits.
