Case Manager, Childrens Rescue Fund
Overview
Lead direct case management services to ensure sustainable permanent housing transition for families experiencing homelessness. Utilize CARES system and collaborate with community resources to meet medical educational and employment needs.
What You'll Do11
- 1Utilize CARES to complete family intake assessments and develop service plans
- 2Review information from HR administration and other agencies to update records
- 3Provide advocacy and referrals for benefits and entitlements
- 4Coordinate stabilization efforts for substance abuse and household management
- 5Assess client needs and develop service plans monitored over 90 days
- 6Maintain relationships with families to promote active participation
- 7Offer counseling and crisis intervention to stabilize families
- 8Facilitate group sessions to enhance life skills and self sufficiency
- 9Coordinate interagency services and communicate updates to staff
- 10Serve on-call weekly and assist in client transfers
- 11Ensure accurate documentation of client interactions and progress
Requirements10
- 1Bachelor’s degree in Social Work or related field with one year experience
- 2Associate degree with two years experience preferred
- 3Experience facilitating groups for homeless adults
- 4Specialized knowledge of substance abuse treatment
- 5Bilingual ability highly desirable
- 6Excellent written and verbal communication skills
- 7Proficiency with Microsoft Office applications
- 8Physical stamina to lift 30 pounds multiple times weekly
- 9Flexibility to travel throughout NYC area
- 10Ability to work long hours with varied schedules
