Overview

You will own the full case management cycle for families in Transitional Housing, guiding them from intake to permanent housing. You will assess needs, build Individualized Living Plans, and connect clients to benefits, healthcare, and legal aid. Working alongside Housing Coordinators and Income Building teams, you will use Trauma-Informed Care and Motivational Interviewing to help families achieve stability. What You'll Do8

  • 1Conduct comprehensive assessments and complete intake documentation for newly arrived families within required timeframes.
  • 2Develop Individualized Living Plans that target housing, employment, income, and family stability goals.
  • 3Deliver case management, crisis intervention, and counseling tailored to each family's needs.
  • 4Connect clients to public benefits, healthcare, mental health services, education, and legal assistance.
  • 5Apply Evidence-Based Practices, Motivational Interviewing, and Trauma-Informed Care to support client success.
  • 6Maintain accurate client records and CARES documentation, ensuring compliance with DHS, OTDA, and Win standards.
  • 7Collaborate with Housing Coordinators, Income Building staff, and Childcare teams to advance client goals.
  • 8Monitor progress, conduct follow-ups, and facilitate referrals to move families toward permanent housing.

Requirements6

  • 1Bachelor's degree in social work or related field with 2+ years of relevant experience, or Associate's degree with 3+ years of client-facing experience.
  • 2Experience supporting individuals and families facing homelessness, domestic violence, or other complex challenges.
  • 3Knowledge of strength-based case management, Motivational Interviewing, and Trauma-Informed Care.
  • 4Familiarity with public benefits, entitlement systems, housing resources, and community-based services.
  • 5Strong organizational, communication, and documentation skills.
  • 6Ability to manage a caseload while balancing multiple priorities.
