Overview
As the on-site Social Worker for our supportive housing program in the Bronx, you own resident stability from move-in through long-term retention, directly impacting a portfolio of 150+ units. Embedded in a collaborative team of property managers and service coordinators, you deliver individualized support that prevents evictions and connects residents to healthcare, benefits, and employment resources. What You'll Do9
- 1Serve as the primary point of contact for residents needing supportive services, conducting LMSW-level assessments and intake interviews.
- 2Develop individualized service plans that address mental health, substance use, and daily living skills, reviewing progress bi-weekly.
- 3Coordinate move-ins and tenant orientations for new residents, ensuring seamless transitions and immediate connection to supportive services.
- 4Connect residents to community resources including SNAP, Medicaid, SSI/SSDI, and employment programs, tracking referral outcomes.
- 5Collaborate with property management to resolve tenancy issues, mediate conflicts, and implement eviction prevention strategies.
- 6Provide crisis intervention for residents in acute distress, using de-escalation techniques and coordinating emergency services.
- 7Manage housing subsidy compliance, including recertifications and rent calculations, in partnership with HRA and HPD.
- 8Maintain timely case notes and documentation in CARES and AWARDS, meeting all regulatory and funding source requirements.
- 9Liaise with government agencies and non-profits to secure ongoing support for residents, attending case conferences and service coordination meetings.
Requirements7
- 1Active New York State LMSW or LCSW license required.
- 2Master's Degree in Social Work (MSW) from an accredited institution.
- 32+ years in supportive housing, affordable housing, or a housing-based program required, with direct resident case management experience.
- 4Strong working knowledge of housing resources, government assistance programs (e.g., Section 8, NYCHA), and tenant rights.
- 5Proven ability to conduct crisis intervention and conflict resolution in a housing setting.
- 6Excellent written and verbal communication skills, with proficiency in electronic health records and case management software.
- 7Capacity to work independently while coordinating with property management and external partners.
