Overview

You will own comprehensive support services for tenants in our supported housing programs, helping them maintain housing and reach personal goals. What You'll Do9

  • 1Facilitate at least four groups per month, either four distinct groups or two recurring ones, to foster community and skill-building.
  • 2Complete all assessments and service plans on time, ensuring they reflect tenant goals and needs.
  • 3Write at least two service plan linked notes per tenant within 48 hours of meetings, documenting progress and outcomes.
  • 4Conduct monthly in-apartment visits for all tenants on your caseload, adjusting to weekly visits in Community Residences.
  • 5Document 40 hours of direct service work monthly, tracking engagement and outcomes.
  • 6Develop and implement individualized support plans using a person-centered model, addressing recovery, community connection, and vocational goals.
  • 7Coordinate access to community-based treatment and support services, advocating for tenant needs.
  • 8Lead group activities that promote social connections and peer support among tenants.
  • 9Maintain confidential records and ensure compliance with program and agency documentation standards.

Requirements9

  • 1High school diploma or GED required; bachelor's degree in a related field preferred, with work experience in the field acceptable as substitute.
  • 22+ years of experience in supportive housing, social services, or mental health settings.
  • 3Proficiency in Microsoft Word and Excel, and ability to learn other computer programs.
  • 4Excellent oral and written communication skills, with ability to document clearly and timely.
  • 5Demonstrated ability to prioritize tasks and meet deadlines in a dynamic environment.
  • 6Skilled in conflict mediation and negotiation, with an assertive approach to problem-solving.
  • 7Ability to work independently and as part of a multidisciplinary team.
  • 8Must be fingerprinted and pass a background clearance, as required by the agency.
  • 9Bilingual in Spanish or other languages preferred, to meet tenant needs.
