Overview
Service Coordinator (Homes Together Program) reports to Program Director. Responsible for case management housing support and monthly reporting to ensure compliance. Attends DHS and NYC Co meetings. Provides case management crisis intervention. Conducts home visits evaluates compliance and refers to supportive services. Takes urine samples for drug screening. Escorts participants to social services agencies. Reports child neglect or abuse. Manages tenant rent calculations inspections and lease updates. Ensure accurate program data.
What You'll Do14
- 1Act as liaison between participants superintendents and building managers to complete repairs
- 2Maintain Building and Apartment Log documenting dates and conditions
- 3Report non-compliance issues to Supportive Housing Manager
- 4Attend monthly DHS and NYC Co meetings
- 5Provide case management and on-site crisis intervention
- 6Complete required service plans
- 7Facilitate monthly CAB tenant meetings
- 8Educate participants on good housekeeping home safety budgeting and landlord/tenant relations
- 9Maintain appropriate documentation of all tenant contacts
- 10Document monthly progress notes in participants' charts
- 11Conduct home visits to evaluate compliance and assess referral needs
- 12Take random urine samples for drug screening toxicology
- 13Escort participants to social services agencies as requested
- 14Report alleged child neglect or abuse to Supportive Housing Manager
Requirements10
- 1Associates degree or equivalent field requirement
- 2Three (3+) years relevant experience in mental or behavioral health environment
- 3Experience working in scattered site or supportive housing
- 4Ability to use business acumen for good judgment and decision making
- 5Ability to deal with sensitive issues with diplomacy and discretion
- 6Strong organizational skills multitasking capability
- 7Excellent interpersonal skills verbal and written communication
- 8Commitment to cultural diversity and sensitivity
- 9Professional demeanor under pressure keen sense of urgency and dedication to quality
- 10Willingness to perform other related duties as assigned
