What You'll Do11
- 1Deliver excellent customer service as a full-time staff member during all operational hours
- 2Welcome and orient visitors families and new participants to staff program and facilities
- 3Gather medical and financial information to establish participant eligibility and complete enrollment process
- 4Develop individualized care plans alongside interdisciplinary team incorporating person-centered planning and participant/family input
- 5Conduct home environment assessments prior to enrollment to evaluate living arrangements and accessibility
- 6Provide counseling education and crisis intervention to participants families ensuring continuity of care
- 7Act as liaison between participants families and social service agencies offering referrals and information
- 8Facilitate support groups for participants families and caregivers
- 9Organize outreach programs for participant recruitment and oversee intake and enrollment process
- 10Maintain accurate and timely documentation including progress notes and treatment plans
- 11Assist in staff training and development as well as internal utilization review committees
Requirements5
- 1Master’s Degree in Social Work Psychology Gerontology or Counseling with 1 year social services experience in aging health or long-term care
- 2Bachelor’s Degree in Social Work with 2 years social services experience
- 3Bilingual Mandarin required Cantonese preferred
- 4Experience in case management with elderly populations
- 5Knowledge of physical mental and social needs of older adults
Salary Insight
$62 - $73k per year
