Social Services Coordinator, Adult Day Health
Overview
Namaste Adult Daycare seeks a compassionate Social Services Coordinator to lead participant support at a Boston based adult day health program. This role drives independence and well being through meaningful activities and culturally inclusive services while ensuring compliance with health standards.
What You'll Do8
- 1Conduct participant intake assessments and gather psychosocial information
- 2Assist participants with social emotional and adjustment related concerns
- 3Participate in interdisciplinary care plan meetings and contribute to goal setting
- 4Connect participants and families to community resources such as transportation benefits programs housing assistance and senior services
- 5Maintain accurate timely documentation including progress notes and care plan updates
- 6Serve as primary point of contact for families and caregivers providing updates and support
- 7Observe and report changes in participant behavior or needs to clinical staff
- 8Support daily program operations participant engagement and special events
Requirements8
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons leading community health initiatives
- 3Bachelor’s degree in Social Work Human Services Psychology or related field
- 4Experience working with older adults adults with disabilities or in a community health setting
- 5Strong communication interpersonal and organizational skills
- 6Ability to maintain accurate records manage documentation and uphold confidentiality
- 7Compassionate patient collaborative teamwork ability
- 8Bilingual English Vietnamese or Chinese preferred
Salary Insight
$60 - $70k per year
