Overview
Part‑time Site Coordinator supports Countryside Adult Day Health Program delivering case management services and group activities. This role drives daily operations and collaborates with the Program Coordinator to ensure quality care.
What You'll Do11
- 1Provide case management services
- 2Develop and lead participant groups
- 3Assist with referral admission and discharge processes
- 4Write quarterly progress notes
- 5Inform families about community resources and make referrals
- 6Attend monthly supervisor meetings with Program Coordinator
- 7Help implement policies and procedures
- 8Conduct record reviews and maintain documentation
- 9Support compliance and policy implementation
- 10Deliver services across multiple Adult Day Health sites
- 11Coordinate with LICSW during supervisor meetings
Requirements5
- 1Bachelor’s degree in Health or Human Services or related field
- 2One year recent social work experience with disabled or elderly adults
- 3Significant experience with physically or mentally challenged adults in community settings
- 4Ability to pass MA state CORI check and CPS check
- 5Company paid medical clearance and pre‑employment physical
