Overview

You will own comprehensive participant care for a panel of 55+ adults in San Diego's PACE program, coordinating medical, social, and home care services to keep them independent. You'll work within an interdisciplinary team, driving care plans and medication management under CMS guidelines. Your role spans clinic and in-home visits, with travel to participant locations. This full-time position offers Monday-Friday hours and a chance to make a deep impact on elder health.

What You'll Do10

  • 1Administer medications via oral, topical, intravenous, and injectable routes in the clinic, documenting every dose.
  • 2Coordinate comprehensive care in interdisciplinary team meetings, including IDT and Plans of Care.
  • 3Conduct initial, semi-annual, annual, and change-of-condition assessments per CMS standards.
  • 4Develop and update nursing care plans that reflect actual services provided.
  • 5Travel to participant homes for visits, including end-of-life admission and in-home nursing beyond LVN scope.
  • 6Manage medication processes: new orders, reorders, pre/post-op meds, pharmacy coordination, and deliveries.
  • 7Facilitate discharge planning from hospitals or skilled nursing, including prescriptions and equipment.
  • 8Maintain medical records and fulfill CESD charting and reporting requirements.
  • 9Collaborate with homecare teams to coordinate supplies, medications, and in-home nursing services.
  • 10Complete monthly medication verification and reconciliation for participants on medication management.

Requirements9

  • 1Current California Registered Nurses License and CPR certification.
  • 21+ year experience with frail or elderly populations.
  • 3IV Certification preferred.
  • 4Valid driver's license and reliable transportation.
  • 5Ability to pass DOJ and FBI fingerprint clearances and drug screen.
  • 6Complete in-service training in first aid and CPR within first six months.
  • 7Adhere to OSHA safety guidelines and infection control procedures.
  • 8Maintain confidentiality of participant and company information.
  • 9Participate in quality improvement initiatives.
