Medical Director Elderly Care
Overview
Medical Director leads medical operations for elderly care organization delivering high-quality patient care through interdisciplinary collaboration. This role focuses on clinical leadership and participant-centered care within a PACE model environment.
What You'll Do13
- 1Conduct comprehensive in-person medical assessments and provide primary care to assigned participants
- 2Develop and implement individualized care plans in collaboration with the interdisciplinary team
- 3Provide ongoing medical oversight and continuity of care
- 4Coordinate admissions and discharges involving hospitals nursing homes acute-care facilities and long-term placements
- 5Collaborate with community-based physicians specialists hospitals and post-acute providers to ensure effective transitions of care
- 6Participate actively in IDT meetings and communicate significant changes in participant condition treatment and care needs
- 7Coordinate and support delivery of appropriate 24-hour medical care
- 8Provide medical oversight for participants residing in nursing facilities including regular visits based upon clinical need
- 9Provide direction guidance and clinical support to Nurse Practitioners and other members of the interdisciplinary team
- 10Promote evidence-based approaches to chronic disease management and geriatric care
- 11Support appropriate utilization of specialists hospital services nursing facilities and other healthcare resources
- 12Maintain a strong focus on quality safety participant experience and clinical outcomes
- 13Participate in a rotating physician on-call schedule
Requirements12
- 1Completion of a physician residency program accredited by the Accreditation Council for Graduate Medical Education
- 2Master's degree or doctoral degree as applicable to the physician's professional credentials
- 3Board certification in Internal Medicine Family Medicine or Emergency Medicine
- 4Geriatric medicine experience training or certification strongly preferred
- 5Active unrestricted physician license in the applicable state
- 6Current state Controlled Dangerous Substances certification as applicable
- 7Current DEA registration
- 8Eligibility for or current enrollment as a provider with Medicare and Medi-Cal
- 9Demonstrated knowledge of chronic disease management and geriatric medicine
- 10Ability to work collaboratively within an interdisciplinary care model
- 11Williness to participate in rotating night and weekend on-call schedule
- 12Experience providing care in multiple settings including PACE centers participant homes hospitals and nursing facilities
