The role owns patient rounds at ALF and board and care facilities while maintaining strong relationships to improve outcomes. What You'll Do9
- 1Round on ALF and board and care residents plus select SNF and home visits
- 2Manage chronic disease medication changes and acute issues on-site
- 3Conduct comprehensive visits including history exam and care planning
- 4Collaborate with facility staff psychiatry palliative care and neurology teams
- 5Maintain accurate documentation in EMR with training provided
- 6Provide real-time care decisions to reduce unnecessary ER visits
- 7Work closely with interdisciplinary teams to coordinate patient services
- 8Adapt to varied settings including SNF and mixed panel environments
- 9Develop clinical judgment and communication skills across diverse populations
Requirements6
- 15+ years experience in geriatrics internal medicine family practice home health hospice or ALF/SNF/board and care/LTC settings
- 2CPC Card or BCBA certification preferred
- 3Medical license in good standing
- 4Valid driver's license and reliable vehicle
- 5Ability to commute between multiple locations
- 6Strong organizational skills and commitment to patient-centered care
