Hospice Medical Director & Team Physician
Overview
You will own medical leadership and direct patient care for the hospice program, driving clinical quality, hospice eligibility, and regulatory compliance across the Minneapolis market. You will join a interdisciplinary team of nurses, social workers, and chaplains, supporting care plans that honor patient and family goals. This dual role combines bedside impact with shaping clinical excellence and advancing hospice care. You will also build referral relationships and mentor hospice physicians, a role that stands out for its blend of clinical and leadership duties.
What You'll Do9
- 1Lead hospice services to ensure high-quality, patient-centered care, delivering on clinical quality and safety metrics.
- 2Drive hospice eligibility determinations and care planning, ensuring accurate certifications and recertifications.
- 3Serve on the Interdisciplinary Group (IDG), contributing to care plans, team discussions, and quality improvement initiatives.
- 4Complete face-to-face encounter attestations and certifications in line with hospice regulations within the first 90 days.
- 5Provide 24/7 on-call coverage, delivering timely clinical support for pain and symptom management.
- 6Mentor and develop hospice physicians, fostering clinical excellence and performance.
- 7Ensure compliance with regulatory, accreditation, and organizational standards, including Medicare requirements.
- 8Build lasting relationships with hospitals, long-term care providers, and referral partners to grow the program.
- 9Deliver direct patient care and clinical consultation across hospice settings.
Requirements10
- 1M.D. or D.O. licensed in Minnesota, with training or experience in chronic or terminal illness care.
- 2Active DEA license and valid driver's license.
- 3Working knowledge of hospice and palliative care, including pain and symptom management.
- 4Understanding of primary medical care principles and Medicare hospice regulations.
- 5Proven clinical judgment and decision-making in complex end-of-life care.
- 6Collaboration with attending physicians and interdisciplinary teams.
- 7Excellent communication with patients, families, and providers.
- 8Preferred: board certification in Hospice and Palliative Medicine.
- 9Preferred: 10+ years of clinical experience.
- 10Preferred: 5+ years in a medical director or administrator role.
