Overview
Own the intake and coordination workflow for home health services at scale within a private agency. Partner with referral sources, field teams, and care management to ensure timely admissions and compliant processes. Work in a hybrid setup in Tempe AZ with direct impact on patient transitions and caregiver alignment. This role is unique for combining intake, case management liaison duties, and cross-department coordination.
What You'll Do8
- 1Build and manage referral intake, ensuring timely documentation and authorization across departments
- 2Schedule and coordinate care with field team and Client Care Management to meet patient needs
- 3Review referral quality, authorizations, and consultation with field professionals to ensure accurate care plans
- 4Ensure referrals include proper hours, staffing acceptance, and appropriate authorization
- 5Coordinate seamless transition of patients to home care and oversee patient education and preparation for home care
- 6Maintain strong relationships with Managed Care Organization case managers for timely communication
- 7Establish and maintain relationships with current and potential referral sources
- 8Respond to member referrals promptly and maintain high level customer service with stakeholders
