Hospice RN Case Manager, North/East Phoenix & North Scottsdale
Overview
Own the case management for terminally ill patients in home and community settings across North/East Phoenix. Lead coordination with the IDG to develop and adjust patient/family care plans within HIPAA guidelines. Collaborate with physicians to monitor and respond to changing patient needs and provide bereavement support. You will educate families and supervise care activities to ensure compassionate, compliant hospice care. This role offers on the road patient visits and a comprehensive benefits package.
What You'll Do6
- 1Build and maintain patient care plans with the IDG and physician input to ensure continuity of care
- 2Assess and reassess patient needs in home settings and respond to unexpected clinical changes
- 3Teach, supervise and counsel patients and family members on home hospice care
- 4Coordinate caregiver schedules and resources to deliver timely hospice services
- 5Document care activities, maintain HIPAA compliance and protect patient confidentiality
- 6Collaborate with the hospice team to ensure adherence to policies and quality standards
Requirements5
- 1Active RN license in Arizona
- 2Experience providing hospice or home health nursing
- 3Ability to travel to patient homes across North/East Phoenix and North Scottsdale
- 4Strong communication skills with patients, families and interdisciplinary team
- 5Commitment to patient privacy and HIPAA regulations
