Hospice RN Case Manager
Overview
We seek a compassionate experienced Hospice RN Case Manager to join our team. This full-time position involves end-of-life care coordination and patient-centered planning. The ideal candidate is a licensed RN with hospice experience and dedication to patient dignity.
What You'll Do11
- 1Conduct comprehensive patient assessments to identify healthcare needs
- 2Develop individualized care plans and update them as needed
- 3Provide skilled nursing care including pain management and medication administration
- 4Coordinate and supervise hospice aides and other healthcare professionals
- 5Communicate effectively with patients families and physicians about patient progress
- 6Monitor patients physical emotional and spiritual well-being to ensure comfort
- 7Collaborate with interdisciplinary team to align care strategies with patient goals
- 8Ensure compliance with regulatory guidelines documentation requirements and facility policies
- 9Provide bereavement support and resources to families and caregivers
- 10Participate in team meetings to discuss care plans and progress
- 11Perform all duties assigned by the Hospice Director or supervisor
Requirements11
- 1Current Registered Nurse license in the state of practice
- 21 year hospice RN experience preferred
- 3Strong clinical assessment and critical thinking skills
- 4Excellent communication and interpersonal abilities
- 5Ability to work independently and manage time effectively
- 6Valid driver's license and reliable transportation for home visits
- 7Current CPR certification
- 8Knowledge of hospice regulatory requirements and documentation guidelines
- 9Commitment to compassionate patient-centered care in hospice settings
- 10Experience with electronic medical record systems preferred
- 11Physical ability to lift transfer patients and carry equipment up to 50 pounds
