Hospice RN Case Manager
Overview
The Hospice RN Case Manager leads patient care coordination in a hospice setting. This full-time position involves developing individualized care plans and delivering skilled nursing services. The role demands strong clinical expertise and collaborative teamwork.
What You'll Do11
- 1Conduct comprehensive patient assessments to identify healthcare needs and develop care plans
- 2Provide skilled nursing care including pain management and medication administration
- 3Coordinate hospice aides and healthcare professionals
- 4Communicate updates to patients families and physicians
- 5Monitor patient well-being and ensure dignified end-of-life care
- 6Collaborate with interdisciplinary teams to align care strategies with patient goals
- 7Ensure regulatory compliance and proper documentation
- 8Offer bereavement support to families and caregivers
- 9Participate in team meetings to discuss patient progress
- 10Perform additional duties as directed by the Hospice Director
- 11Maintain vehicle for home visits and adhere to scheduling requirements
Requirements11
- 1Current RN license in the state of practice
- 2Minimum two years hospice RN experience
- 3Strong clinical assessment and critical thinking abilities
- 4Excellent communication and interpersonal skills
- 5Ability to work independently and manage time effectively
- 6Valid driver's license and reliable transportation
- 7Current CPR certification
- 8Knowledge of hospice regulations and documentation standards
- 9Commitment to compassionate patient-centered care
- 10Experience with electronic medical records preferred
- 11Physical capability to lift 50 pounds and stand for extended periods
