Overview

What You'll Do11

  • 1Conduct comprehensive patient assessments to identify healthcare needs and develop care plans
  • 2Provide skilled nursing care including pain management and medication administration
  • 3Coordinate hospice aides and healthcare professionals
  • 4Communicate updates to patients families and physicians
  • 5Monitor patient well-being and ensure dignified end-of-life care
  • 6Collaborate with interdisciplinary teams to align care strategies with patient goals
  • 7Ensure regulatory compliance and proper documentation
  • 8Offer bereavement support to families and caregivers
  • 9Participate in team meetings to discuss patient progress
  • 10Perform additional duties as directed by the Hospice Director
  • 11Maintain vehicle for home visits and adhere to scheduling requirements

Requirements11

  • 1Current RN license in the state of practice
  • 2Minimum two years hospice RN experience
  • 3Strong clinical assessment and critical thinking abilities
  • 4Excellent communication and interpersonal skills
  • 5Ability to work independently and manage time effectively
  • 6Valid driver's license and reliable transportation
  • 7Current CPR certification
  • 8Knowledge of hospice regulations and documentation standards
  • 9Commitment to compassionate patient-centered care
  • 10Experience with electronic medical records preferred
  • 11Physical capability to lift 50 pounds and stand for extended periods
