What You'll Do13
- 1Conduct initial and ongoing comprehensive assessments of terminal disease impacts across physical functional psychosocial and environmental domains through documentation and clinical records
- 2Apply admission and re-certification criteria to determine hospice eligibility and appropriate care levels
- 3Develop implement and document care plans ensuring quality continuity and recommending revisions as needed
- 4Educate patients families on disease processes self-care nutrition and end-of-life care
- 5Prioritize needs with interdisciplinary team members and provide training to support staff
- 6Determine service scope frequency based on acuity and patient/family requirements
- 7Assess caregiver ability to meet immediate needs and initiate preventive rehabilitative procedures
- 8Apply infection control and standard precautions to prevent disease transmission
- 9Provide clinical directions to hospice aides and licensed practical nurses
- 10Prepare detailed clinical notes demonstrating progress toward established goals
- 11Monitor cases to ensure third-party payer compliance and evaluate patient responses
- 12Demonstrate commitment professional growth and agency philosophy adherence
- 13Promote organizational policies and foster effective communication across all stakeholders
Requirements5
- 1Current RN license with good standing
- 2Experience using Homecare Homebase EMR preferred
- 3Strong clinical assessment skills
- 4Excellent communication and interpersonal abilities
- 5Ability to work independently and manage time effectively
Salary Insight
$75 - $90k per year
