Halcyon Home seeks a compassionate Registered Nurse to lead hospice care for our growing geriatric population in Dallas. You will own patient assessments and care plans while collaborating with a supportive team. This full-time position offers competitive compensation and benefits.
What You'll Do11
- 1Conduct comprehensive initial and ongoing assessments of patient physical functional and psychosocial needs documenting all findings.
- 2Apply admission and re-certification criteria to determine appropriate hospice eligibility levels.
- 3Develop and implement care plans ensuring quality continuity and recommending adjustments as needed.
- 4Communicate effectively with patients families and interdisciplinary teams about patient status and interventions.
- 5Follow infection control protocols during patient interactions to prevent disease spread.
- 6Provide clinical guidance to hospice aides and licensed practical nurses to maintain service standards.
- 7Prepare detailed clinical notes demonstrating progress toward established goals.
- 8Monitor patient cases to verify compliance with third-party payer requirements.
- 9Educate patients and families on disease processes self-care nutrition and end-of-life care practices.
- 10Prioritize urgent needs within the IDG team structure.
- 11Train additional staff on required procedures and protocols.
Requirements5
- 1Current RN license in good standing with state board.
- 2Experience using Homecare Homebase EMR preferred.
- 3Strong clinical assessment skills for geriatric patients.
- 4Excellent communication and interpersonal abilities.
- 5Ability to work independently and manage time effectively.
Salary Insight
$42 - $44k per year
