Registered Nurse Case Manager, Hospice
Overview
As a Registered Nurse Case Manager at Angel Hands Hospice, you will own patient care plans and coordinate interdisciplinary teams across Dallas, Texas. You will perform nursing assessments, provide education and emotional support, and ensure dignity and comfort for each patient. What You'll Do7
- 1Coordinate hospice patient care plans from admission to discharge, updating them weekly with the team.
- 2Perform comprehensive nursing assessments, identifying symptom management needs within 24 hours of admission.
- 3Collaborate with physicians, aides, social workers, chaplains, and caregivers to adjust care plans as conditions change.
- 4Educate patients and families on end-of-life care, medications, and disease progression with empathy.
- 5Document all care accurately and timely using EClinicalWorks and paper charts, meeting regulatory standards.
- 6Lead weekly interdisciplinary team meetings to review patient status and update goals.
- 7Provide after-hours triage support to patients and families, coordinating emergency visits when needed.
Requirements6
- 1Active Texas RN license in good standing.
- 22+ years of hospice or home health experience preferred.
- 3Strong assessment and communication skills, with the ability to build trust quickly.
- 4Compassionate, dependable, and self-motivated with a focus on patient-centered care.
- 5Reliable transportation and a valid driver's license; travel within Dallas county required.
- 6Ability to work independently while collaborating with an interdisciplinary team.
