Rn Case Manager, Hospice Care & Interdisciplinary Team Coordination
Overview
The RN Case Manager will own comprehensive patient assessments and care planning for hospice patients at scale across home settings. This role collaborates with physicians, social workers, chaplains, and aides to deliver patient centered care while ensuring regulatory compliance and quality outcomes. You will lead care plan development, coordinate discharge planning, and provide emotional support to patients and families.
What You'll Do10
- 1Build and implement individualized care plans with the IDT to drive quality patient outcomes
- 2Coordinate home visits, monitor conditions, and adjust care plans to meet changes in patient status
- 3Educate patients and families about disease progression, medications, and end of life care
- 4Serve as primary point of contact for patients and families and provide emotional support
- 5Participate in IDT meetings and contribute to care planning discussions
- 6Ensure compliance with Medicare, Medicaid and hospice regulations
- 7Coordinate discharge planning when appropriate
- 8Document care according to regulatory and agency standards
- 9Provide skilled nursing care focused on pain and symptom management
- 10Monitor and report regulatory and quality metrics to leadership
Requirements5
- 1Active Registered Nurse license
- 2Experience in hospice, palliative, or end-of-life care
- 3Ability to travel for home visits and work hybrid schedule in Phoenix, AZ
- 4Strong communication and family counseling skills
- 5Familiarity with Medicare, Medicaid, and hospice regulations
