What You'll Do5
- 1Build compassionate patient care plans by coordinating with the Clinical Director and interdisciplinary team
- 2Design and deliver direct patient care to uphold comfort and quality of life in the home or facility setting
- 3Educate and advocate for patients and families to align care with goals and preferences
- 4Respond to clinical referrals with timely, empathetic communication and documentation
- 5Share on-call responsibilities to ensure care continuity during evenings and weekends
Requirements5
- 1Current unencumbered RN license
- 2Minimum 1 year RN experience in medical-surgical or acute care setting
- 3Hospice experience a plus
- 4Proficient computer skills and EMR familiarity
- 5Current CPR certification if state required
