Overview
Own direct patient care in hospice with end of life focus at scale within a collaborative team. You will follow the plan of care under RN supervision and contribute to positive patient outcomes. This role sits onsite in Phoenix and offers full-time hours with generous benefits.
What You'll Do5
- 1Provide direct patient care according to the plan of care and state nurse practice act
- 2Document patient services accurately and timely to reflect the plan of care
- 3Educate patients and families on diagnoses and self care plans
- 4Coordinate hospice services by communicating needs to the interdisciplinary team
- 5Manage pain and symptom relief and assess patient response to treatments
Requirements5
- 1Graduate of an accredited practical nursing program
- 2Three years nursing experience
- 3Current LPN license in the state
- 4Valid driver's license with insured vehicle
- 5Strong communication and organization skills
