What You'll Do9
- 1Build and maintain therapeutic relationships with patients and families

- 3Lead daily care activities including medication administration

- 5Collaborate with interdisciplinary teams to coordinate holistic care
- 6Drive process improvements in patient flow and treatment efficiency
- 7Support bereavement services coordination for grieving families
- 8Maintain accurate medication inventory and supply chain management
- 9Train and mentor temporary nursing staff on hospice protocols
Requirements11
- 15+ years working as a Licensed Vocational Nurse or Practical Nurse
- 2Current state license and graduation certificate from accredited program
- 3Valid driver’s license with reliable vehicle
- 4Two years clinical nursing experience preferred
- 5Hospice or home health background demonstrates ideal fit
- 6Proficiency with EMR systems and medical terminology
- 7Effective communication skills for patient and family education
- 8Strong organizational abilities for managing multiple patient assignments
- 9Dedicated attitude toward palliative care philosophy
- 10Ability to work flexible weekend schedules with minimal advance notice
- 11Commitment to maintaining confidentiality and professional boundaries
