What You'll Do10

  • 1Build and manage a primary caseload delivering skilled hospice nursing care
  • 2Coordinate services for hospital discharges to home or facilities and develop plans of care
  • 3Supervise home health aide and LPN care delivery and document outcomes
  • 4Lead interdisciplinary team discussions and contribute to IDG meetings and care conferences
  • 5Ensure timely documentation of medication regimens and care coordination within 24 hours
  • 6Assess home safety and educate patients and families on symptom management and end-of-life care
  • 7Maintain compliance with participation requirements and third-party payor policies
  • 8Support patients, families, and caregivers with bereavement assessments and emotional support
  • 9Communicate with physicians, facilities, and social workers to address holistic patient needs
  • 10Travel extensively within service area to provide frequent in-home nursing care

Requirements5

  • 1Graduate of an accredited nursing program and current RN license in Indiana
  • 2Minimum 1 year of nursing experience in a clinical care setting
  • 3Reliable transportation and willingness to travel extensively within service area daily
  • 4Excellent communication skills and ability to build rapport with patients and families
  • 5Ability to work independently and manage multiple priorities in varied environments
